Death Notifications

Death notices may be sent via regular mail or by e-mail.

 Please include the following information in the Death Notification:

• Member's full name
• Member's Military Unit or Branch of Service, etc.
• Date of Death
• Next of Kin's Full Name
• Next of Kin's Street Address or P.O. Box
• Next of Kin's City, State and ZIP

This information is needed for a report to the Freedom Foundation in Valley Forge.  It is also important for maintaining correct Membership Records, for the Quan mailing list, and for the Memorial Certificate Program.
